Workplace Investigation Report (MA)

A workplace investigation report is used to document inappropriate behavior, safety issues, policy violations, or any other type of workplace misconduct.

This form ensures all relevant details are recorded, including incident descriptions and employee and witness statements. This facilitates decision-making regarding disciplinary actions and helps maintain compliance and fairness in handling workplace issues.

This form should be reviewed together with your employee handbook and other existing company policies. It's designed to be used with the Progressive Discipline Policy, Written Warning, Final Disciplinary Notice, and Termination Notice. This is not intended for employees covered by a collective bargaining agreement.
