This Remote Work Policy lays out basic ground rules and expectations for remote workers, such as having a dedicated workspace, maintaining confidentiality, and adhering to designated work time. It can be customized to include if the company will provide equipment and information on permitted break time. There is also a place at the end for the employee to sign confirming that they have read and understand the agreement.

This policy should be reviewed in conjunction with your employee handbook and other existing employee policies to ensure there aren't any inconsistencies.
